To make your appointment easier and save your time in our office, Canton Dental Clinic has provided our patient forms in a downloadable format. We encourage you to download and complete your patient forms before your appointment with Dr. Laura Pranke, Dr. Caleb Mckinley, and Dr. Melissa Brandner, and to bring it with you when you visit our dentists for the first time.
